Zhodnocení technické efektivnosti nemocnic v České republice

Abstract

Cílem diplomové práce je zhodnocení technické efektivnosti vybraných čtrnácti nemocnic České republiky za období 2018-2022. Analýza je provedena pomocí vstupně orientovaných modelů s využitím metody DEA, které předpokládají konstantní a variabilní výnosy z rozsahu. V souvislosti s cílem diplomové práce byly stanoveny dva dílčí cíle a dvě výzkumné otázky VO1 a VO2. Výzkumná otázka VO1 zní: „Jsou rozdíly v technické efektivnosti vybraných nemocnic před pandemii COVID- 19 v ČR, tedy v letech 2018–2019 a v jejím průběhu, v letech 2020-2022?“ a VO2 „Nemocnice nad 1500 lůžek dosahují lepší technické efektivnosti než nemocnice, které mají počet lůžek v rozmezí 500-1000 lůžek a 1001-1500.“ Pro ověření stanovených výzkumných otázek byly formulovány dva model (M1, M2), s využitím 4 vstupních parametrů (počet lůžek, počet lékařů, počet sester, počet všech zdravotnických pracovníků) a výstupních parametrů (počet hospitalizovaných pacientů, počet ambulantních pacientů). Na základě dosažených výsledků bylo zjištěno, že vybrané nemocnice byly efektivnější před pandemií COVID-19, zatímco v jejím průběhu dosahovaly nižších výsledků průměrné technické efektivnosti. Z hlediska velikosti lůžkového fondu dosahovaly nemocnice s nižším počtem lůžek (500-1000) lepších výsledků než nemocnice, které měly více než 1500 lůžek, ale vzhledem k malému počtu hodnocených jednotek (4 jednotky) nelze jednoznačně potvrdit nebo vyvrátit VO2. Nemocnice s počtem lůžek v rozmezí 1001-1500 dosahovaly horší technické efektivnosti než nemocnice s nejnižším počtem lůžek.

The aim of the thesis is to evaluate the technical efficiency of selected fourteen hospitals in the Czech Republic for the period 2018-2022. The analysis is performed using input-oriented models with the DEA method, which assume constant and variable returns to scale. Two sub-objectives and two research questions VO1 and VO2 were set in relation to the aim of the thesis. Research question VO1 is: "Are there differences in the technical efficiency of selected hospitals before the COVID- 19 pandemic in the Czech Republic, i.e. in 2018-2019, and during the pandemic, i.e. in 2020-2022?" and VO2, "Hospitals above 1500 beds have better technical efficiency than hospitals that have a bed count between 500-1000 beds and 1001-1500 beds." To test the set research questions, two models (M1, M2) were formulated using 4 input parameters (number of beds, number of doctors, number of nurses, number of all health care workers) and output parameters (number of inpatients, number of outpatients). Based on the results obtained, it was found that the selected hospitals were more efficient before the COVID-19 pandemic, while during the pandemic they achieved lower average technical efficiency results. In terms of bed size, hospitals with fewer beds (500-1000) performed better than hospitals with more than 1500 beds, but due to the small number of units evaluated (4 units), it is not possible to clearly confirm or refute VO2. Hospitals with bed counts in the range of 1001-1500 achieved worse technical efficiency than hospitals with the lowest bed counts.
