Abstract

This thesis deals with the complete development of database information system. It means that it starts from the submission, through concept, to implementation, containing testing and implementation of the base system into operation. The implementation is the main target together with commissioning of a web application that is tasked to record the patients symptoms of a disease called chlamydia, and then shape graphs for better treatment for patients in hospitals. The system is the first of its kind and is designed so that it can be supplemented by other necessary functions. The work is divided into three parts. The first chapter deals with what are chlamydia, how the disease manifests itself in patients, what their treatment is and what species are known to us. The second chapter describes the design of information systems and the way of implementation of Web applications is described in the last part.
