Prostorová distribuce QRS komplexu 12 svodového EKG u pacientů podstupujících implantaci přístroje pro srdeční resynchronizační terapii

Abstract

The thesis deals with advanced analysis of preimplantation and postimplantation ECG recordings in patients indicated for cardiac resynchronization therapy. The main focus of the thesis is the use of signal decomposition method to singular values in this cohort of patients. The introductory research develops the topic as as a whole and prepares the author and the reader for the practical part that follows. The latter focuses on the analysis of selected parameters, namely QRS complex width and QRS complex microfragmentation value, which undergo an acute change after pacemaker implantation. According to the terms of reference of the thesis, a complex analysis was performed, from the preprocessing of ECG signals to the calculation of both mentioned parameters in individual patients. Comparison of the values of both types of predictors in preimplantation and postimplan- tation records was performed. On the basis of the obtained results it was proved and verified that after implantation of the pacemaker there is a significant reduction of both mentioned parameters. Furthermore, the relationship between the two types of predictors was examined in more detail. As a result, it was confirmed that the QRS microfragmentation parameter is a completely independent predictor in this cohort of patients.
