Návrh a tvorba prediktivního modelu pro hodnocení míry rizika léčby u chronické lymfocytární leukemie

Abstract

This diploma thesis deals with the design of a predictive model for assessing the degree of risk of treatment in chronic lymphocytic leukemia. Currently, over 75% of patients are diagnosed at an early stage (Binet A, Rai 0) and therefore do not need treatment, they are only monitored. However, about half of these patients are gradually deteriorating and need treatment in the future. The course of the disease in patients is individual and their probability of needing treatment is hardly predictable. Creating a prognostic model for the first treatment in these patients, who are at high risk of disease progression, would be very beneficial. In the theoretical part I deal with the extension of AIPS-E (alternative international prognostic scoring system) by other factors, such as age over 65 years, complex karyotype (CK), deletion of 6q21, trisomy 12, IgH and deletion of 13q14, which could refine prognostic scores to TTFT for early phase CLL. Data for patients from the Olomouc University Hospital in whom CLL was diagnosed were provided for statistical processing and for model design. For practical use, a predictive model was proposed to assess the degree of risk of treatment in chronic lymphocytic leukemia.
